Your Care Team:
Medical Director: Alongside your Primary Care Doctor, our medical director collaborates on the care needs for every patient based of the collective thoughts of the care team.
Nurse Case Manager: Every patient is assigned an RN case manger, who will usually visit one to two days per week. Additionally, our nurses will make visits if a patient is uncomfortable. We also have an on-call team available 24 hours a day 7 days a week.
Medical Social Workers: Our social workers assist patients and families with their emotional and social needs.
Chaplain Services: Hospice Chaplains provide emotionl and spiritual support. Our goal is to affirm life and always meet patients and families where they are. Our Chaplains always provide excellent companionship.
Home Health Aides: Hospice aides are assigned to patients based on the needs of the patient and caregiver.
Volunteers: Hospice Volunteers provide ongoing companionship to hospice patients by reading, listening to music, hand massage, plus many additional requests.
