Hospice Services are provided for patients with a life limiting illness. It includes the following services:
Care Team: The Care Team consists of several disciplines to suppor the physical, emotional, and spiritual needs of any hospice patient. Collectively as a team, we develop individualized plans of care for each patient.
Medical Equipment: Hospice will cover various durable medical equipment to promote optimal wellness for our patients. (Examples: Hospital Beds)
Medications: At Sunset Hospice, all medications related to a patients terminal illness are covered under their hospice benefit.
Funeral Planning: Our Social Workers are available to assist all patients and families with funeral planning needs. We provide many resources to support your decisions.
Bereavement Services: Our agency offers several bereavement support options 13 months following the death of a loved one. We understand that grief comes in many forms and we are here to support you.
